PREPARING FOR YOUR NEW PATIENT/ INITIAL VISIT

Please review the following helpful information to prepare for your first visit:
Please bring your insurance ID card and your driver's license with you. We will need to copy these for your file.
We need to verify your insurance prior to your visit with the doctor. You may fax your information to us ahead of time if you desire. (If doing this, please fax your registration form and an enlarged copy of your insurance card - front and back - to 281-807-5311. Please write the date of your visit on the fax. You still will need to bring your original forms with you on the day of your visit). Providing your information ahead of your visit will save you time on the day of your visit.
If we do not participate with your insurance plan or are not able to verify benefits, payment will be expected at the time of service. Also, any copays or deductibles are due at the time of service.
Please arrive at least 15-20 minutes ahead of your scheduled appointment time. The staff needs to prepare your chart for the doctor and ensure your information is complete. If you are late, you may be asked to reschedule.
We reserve extra time for our new patients. Please call at least 24 hours in advance to cancel if you are not able to keep your scheduled appointment.
On the Health History form is a place to list your current medicines. Please be as complete as possible, including any over-the-counter medicines or supplements that you take. You may choose to use a separate page to list your medicines and provide this to us at your viist. If you are going to need refills, please be sure to let us know at the time of your visit.
